About the Seniors Lifestyle Victoria program
Seniors Lifestyle Victoria is a free program that helps older Victorians to stay active, stay strong and stay connected. The program helps participants to navigate the healthcare system so they can stay independent, healthy and well.
This might include support services focused on:
- physical activity and nutrition
- medication management
- access to health professionals, including mental health support
- social connection
Participation in the Seniors Lifestyle Victoria program is free for eligible participants. The program will be available until June 2028.
Who can take part in the program
This program is for people:
- aged 65 years or older, or 50 years or older if they are Aboriginal or Torres Strait Islander people
- living at home (including with a friend or family member)
- who would benefit from support including those who:
- are finding it harder than before to get around or do everyday things like shopping, cooking, cleaning or other activities they used to enjoy
- have had a fall in the last 12 months
- in the last 6 months, have sought help for an unplanned health issue through Nurse-on-Call, an ambulance, an urgent care clinic, or hospital (including the emergency department)
People who meet the above eligibility criteria may also include those who:
- have recently been discharged from hospital or are about to be discharged from hospital
- are experiencing social isolation and/or loneliness
- from culturally and linguistically diverse backgrounds and/or have limited verbal and written skills in English
- have multiple other health and/or mental health comorbidities
- are at risk of poor health outcomes due to economic and social impacts.
How to get involved in the program
- A doctor can refer you to the Seniors Lifestyle Victoria program.
- You can also refer yourself or someone you care for by contacting one of the participating community health services.
The program is available through selected community health services who deliver this program in metropolitan and regional Victoria.
The program will be available at the following community health services by early 2026.

Participation in the program is free.
